Codex follows Polygon’s finalized head, a few seconds behind the tip, so nothing it serves is ever rolled back. Polygon makes a block about every 1.5 s. Every answer tells you how far it reflects the chain:
The live tape, /v1/stream/fills, is the exception: it publishes fills at the chain head, before finality, so it can repeat a block after a reorg. Deduplicate its fills by (tx, logIndex).
